WMCHealth PPS is offering a Care Coordination Training for Front Line Workers (Unlicensed) 

Description: Free Training for WMCHealth PPS partners and staff. Registration required. This program provides fundamental knowledge regarding care coordination standards and principles required for healthcare services transformation.

Part I. In-person training. Participants must choose one date:

       \ü  Date/Time: February 9th 2018. 9:00 a.m. – 5:00 p.m.

 ü  Date/Time: March 23 2018. 9:00 a.m. – 5:00 p.m

o   Location: Health Alliance Hospital. Broadway Campus Conference Room A. 396 Broadway Kingston, NY 12401

 Course delivery: Course participants will complete 1 in-person class and 7 online courses.

Part I:  Care Coordination In person training -1 full day 

Part II: Online webinars and e-courses *Students will receive a certificate upon completion.

Who should attend?

·         Front line personnel from PCPs/primary care teams, behavioral health care teams, community-based providers (including both Medicaid billing and non-Medicaid billing); FQHCs care teams, and relevant hospital staff (unlicensed).

·         Roles:  Including but not limited to personnel who provide care coordination functions in multiple settings and roles across the WMCHealth PPS network; non-licensed care coordinators, case managers, care navigators, community health workers, peer support workers, Medicaid service coordinators, and health educators.
